How has the Form 800SR changed?We included the required disclosure and training on one form so Investigators do not need to complete the COI disclosure form and training certification separately. We also reduced the financial interest disclosure questions from four to only one. Who is this form for?This form applies to Investigator(s) from a subrecipient entity without an active and enforced conflict of interest policy consistent with the PHS regulations. Additionally, all other non-UCI consultants, collaborators and others identified as Investigators but who are not contributing to the project under a subrecipient agreement must also complete this form. Who is considered an “Investigator” and, therefore, required to disclose?“Investigator” describes a project director or principal Investigator and any other person, regardless of title or position, who is responsible for the design, conduct, or reporting of research funded by the PHS, or proposed for such funding, which may include, for example, collaborators or consultants. Can I start using the new form now?Yes!In fact, we encourage you to start using the new forms as soon as possible. If we see an old form being used after the official implementation date of August 31st, we may request that you complete a new form. To avoid this extra work, get ahead by using the new Form 800SR for all non-UCI Investigators and subrecipients following UCI’s COI policy.
